Should I schedule a physical therapy or wellness appointment?

If you recently sustained an injury/accident or have a chronic condition that has flared up or has not been alleviated by traditional means, please schedule an initial Physical Therapy Evaluation. If you are looking for overall health and wellness including improved mobility, flexibility, stability, sport performance as well as education and guidance on lifestyle habits and stress management, please schedule an initial Wellness Evaluation. Many physical therapy clients will transition into wellness as their symptoms improve. There are elements of overlap between both programs. If you are still uncertain, please contact us.

Do you take my insurance?

At Foundation Physical Therapy and Wellness, we value YOU. We aren’t looking to “do physical therapy.” We are looking to transform lives. We are looking to help you get back to being active and thriving in your full potential! We are looking at you as a whole person, not as your injury or pain. Therefore, we pride ourselves in serving you with a licensed physical therapist for your entire treatment while personalizing your care. Unfortunately, accepting insurance would restrict our ability to provide you with the type of quality treatment we offer. We can, however, provide you with a superbill that you can submit to your insurance company for out of network benefits. It is your responsibility to know if this is a benefit that you have in your policy. If you are looking for the one on one attention you deserve and/or traditional therapy has not resolved your symptoms, please don’t hesitate to come see us!

What should I wear to my appointment?

We ask that you wear loose, comfortable clothing that you can exercise in. It is also important that we have access to the body region of complaint.

What is your cancellation policy?

If you need to cancel your appointment, we kindly ask that you provide us with a full 24 hours notice from the time of your scheduled appointment. Each time slot is very valuable and less than 24 hours notice does not allow us enough time to fill your spot. A cancelation fee will apply to any cancelations less than 24 hours in advance. We appreciate your understanding.
